Event Description

At a single alarm structure fire where a Firefighter stepped backwards off a 2-story roof while picking up a hose line. The fire was out and the overhaul was complete. Other members were busy with tasks to bring the company off the roof. After talking with the crew and during the debrief, it was determined that the FF was not aware of where the edge of the roof was and others in the crew were focused on their assigned tasks. The FF was transported to the hospital where he was examined by a physician. No serious injuries were found. It was felt by the Officers and all crew members that this was a serious event that could have resulted in a fatal injury.

Lesson Learned

Lessons learned were: 1. Company officer awareness of all crew members during an incident is vital in routine situations. 2. Other crew members shall watch out for others. 3. Provide lighting. 4. At roof operations define area of operation. 5. Officers shall be aware of members' physical abilities throughout the shift.
